The surgical decision is the one that carries the most weight — specifically the choice between lumpectomy and mastectomy, and between unilateral and bilateral. Women describe going back and forth for weeks. What makes it hardest is that the decision is irreversible, it arrives when the capacity to think clearly is lowest, and the right answer depends on things only the patient knows about herself.
